As required under federal law, The Christ Hospital is providing you with several important noticed related to your health plan benefits as follows:

| Exchange Notice | Notice providing information about health coverage available through Exchanges (Marketplaces). |
| HIPAA Privacy Notice | Notice that describes your rights, the Plan’s legal duties with respect to protected health information (PHI), and the Plan’s uses and disclosures of PHI. |
| HIPAA Special Enrollment Notice | Notice of your rights to enroll mid-year upon loss of other coverage, addition of a new dependent, or eligibility for CHIP premium assistance subsidy. |
| Summary Annual Report (SAR) 2022 Plan Year | Summary of financial information reported on our required financial filings. Will be available on or before 12/15/2024 |
| Women’s Health and Cancer Rights Act (WHCRA) Notice | Notice of benefit rights under the WHCRA and any deductibles and coinsurance limits applicable to such benefits. |
| The Newborns’ and Mothers’ Health and Protection Act (NMHPA) Notice | Notice of benefit rights under the NMHPA (providing for minimum length of hospital stay in connection with childbirth). |
| Children’s Health Insurance Program (CHIP)/Medicaid Notice | Notice of opportunity for premium assistance for TCH medical plan to eligible employees residing in states with premium assistance programs under Medicaid or CHIP. |
| Wellness Program Disclosure | Notice explaining your rights if you are unable to participate in any of the health-related activities of the wellness program. |
| Statement of ERISA Rights | Notice that describes your rights and the Plan’s legal duties for benefits that fall under ERISA. |
| Medicare Part D Notice* | Notice stating whether prescription drug coverage is or is not credible. |
